Sometimes the eye is struck so that the force of the blow is received by the eyeball and not blocked by the strong bones around the eye (as when struck by a small object such as a golf ball). In this case, the pressure on the eyeball is transmitted to the walls of the orbit. This pressure can fracture the most fragile part of the orbit, typically the part underneath the eyeball (orbital floor). This is known as a blowout fracture. Fractures can also occur to the sides (walls) and roof of the orbit. Sometimes part of the eye or the muscles attached to it is forced through the fractured bone.

A #mnemonic DECA helps you to memorize Classic Signs of Blow-Out Fracture

• Decreased upgaze (inferior rectus trapped)
• Enophthalmos
• Cheek Anesthetized (infraorbital nerve trapped).

Nosebleeds, swelling of the eyelid, and the passage of blood from ruptured blood vessels into the tissue under the skin (marked by a purple discoloration of the skin) may also occur.

If an orbital fracture is suspected, perform a CT scan, which shows any fractures, collections of blood, and displaced or trapped tissue.
Medical treatment is warranted for patients for whom surgery is not indicated. This may include patients who present without significant enophthalmos (2 mm or more), a lack of marked hypo-ophthalmic, absence of an entrapped muscle or tissue, a fracture less than 50% of the floor, or a lack of diplopia.
