The Financial Scope: SADC centers in Queens capture roughly $2.1 billion annually in Medicaid managed-care disbursements. Recent DOJ actions include a $120 million criminal indictment against Queens operators (Royal Adult Daycare and Happy Life Inc.) and a $68 million Brooklyn conviction, with single storefront facilities often billing between $5 million and $13 million per year.
The Roster Exploitation: Recruiters enrolled 3,000 to 8,000 low-income seniors per facility—far beyond physical capacity (often 50 to 100 seats)—billing government plans roughly $1,600 per person monthly for supervision and services that were never actually delivered ("ghost attendance").
The Kickback Mechanism: Beneficiaries handed over their Medicaid cards and signed falsified attendance sheets in exchange for monthly cash payouts, grocery gift cards, and over-the-counter credit allowances.
