Nachbemerkung

"Although we did not include a control group without prior SARS-CoV-2 infection, as we could not definitively establish the absence of asymptomatic COVID-19 or absence of infection-related SARS-CoV-2 antibodies in a broadly vaccinated population, we compared results with pre-pandemic normative data for corneal subbasal nerves57,58,59, mature DC/T cells60,61, and central corneal esthesiometry62. Normative data in healthy individuals revealed values at the same levels as in our control group, indicating the neural and inflammatory changes observed are not normally present in a healthy population, and are evident in POS but not following COVID-19 generally.

Limitations of our study are its size and lack of overall prevalence estimates for POS following mild SARS-CoV-2 infection. With post hoc power testing, however, at an effect size of d = 0.6, 91% of parameters achieved ≥ 80% power, and all parameters achieved ≥ 78.4% power, indicating the study was sufficiently powered to detect moderate-to-large effects, whereas smaller effects would require larger populations to detect. Also, we relied on self-reported symptoms, which provided limited data on concurrent long-COVID, and we did not acquire detailed data on symptom severity, viral variants, or type and frequency of vaccination"

In Kürze:

Dear Michael,

Thank you for taking the time to read our review.

I think we fundamentally disagree on what the study by Marks et al. is actually able to demonstrate.

The experimental design of Marks et al. is centered on RNA delivery, antigen expression and the subsequent adaptive immune response. Accordingly, essentially all readouts quantify expression, antigen-specific immunity or downstream inflammatory responses. The study was not designed to quantify the relative contribution of antigen-derived versus LNP-derived biological effects:

A central limitation of the study is the absence of a comprehensive empty LNP control arm for the mechanistic endpoints used to attribute biological effects to antigen expression. The supplementary characterization demonstrates that the RNA-LNP formulations are physically comparable and functionally competent, but these analyses do not address the contribution of the LNP component itself to immune activation or pathology.

Without direct comparison of RNA-LNPs with matched empty LNPs across the relevant biological readouts, the study cannot distinguish antigen-dependent effects from responses triggered by the delivery platform. Therefore, the data demonstrate the consequences of RNA-LNP administration but cannot establish that antigen-specific immunity is the predominant mechanism responsible for the observed effects.

For most key endpoints (adaptive immune response, antibodies, tumor control), the study design does not allow for a direct distinction between the portion of the response attributable to the adjuvant effect of the LNP shell itself versus that attributable to the expressed mRNA protein–since the comparison is usually between RNA.WT and RNA.miRT (both with intact LNP) or between RNA and PBS. Only for the liver toxicity data in Fig. 5b–d and the biodistribution baseline measurement in Extended Data Fig. 2 was a true “LNP-only” control included.

For this reason, I do not see how the study can support the conclusion that the immune response to the foreign antigen is the predominant mechanism of harm. At most, it demonstrates that antigen expression can contribute to pathology under the chosen experimental conditions. It can not exclude alternative LNP-driven mechanisms because these were not investigated.

I find this assessment difficult to reconcile with the breadth of evidence currently available. Our review deliberately integrates independent lines of evidence, including consistent omics signatures, functional studies using empty LNPs, and mechanistic data on membrane-active ionizable lipids, that, taken together, support the plausibility of an antigen-independent contribution. One may disagree with this interpretation, but I would not characterize the evidence base as weak.

Moreover, this interpretation seems to be difficult to reconcile with several independent observations. Mori et al. (2026) reported myocarditis following exposure to empty LNPs and identified mitochondrial vulnerability as a central mechanism. Sadly this paper appeared after we had already submitted and were finished with the peer review. In addition, as quoted in our work, Qin et al. demonstrated that exposure to mRNA-LNPs resulted in inheritable alterations of innate immune fitness, indicating biological effects that extend beyond antigen-driven immune responses. Furthermore, Connors et al. showed that empty LNPs alone activate and mature antigen-presenting cells, including upregulation of CD40 and induction of IL-6, IL-12 and IL-21, in the complete absence of an encoded antigen.

Taken together, these findings indicate that biologically relevant LNP-mediated effects cannot be reduced solely to antigen expression. Therefore, I do not see how Marks et al. provide evidence against our hypothesis; rather, they address a different mechanistic question.

Furthermore, several aspects need to be considered regarding the Patisiran issue:

You write: "....and they don't come close to making up for the much higher LNP dose given with patisiran."

As we point out in our review, the comparison with patisiran(Onpattro®) requires caution. The relevant biological parameter is not simply the administered lipid mass. Lipid mass is not equivalent to the number of lipid molecules, particle number, surface-exposed ionizable lipid species, or biological activity of the formulation. Different ionizable lipids can exhibit substantially different interactions with membranes, proteins and intracellular pathways despite differences in total lipid dose. Therefore, a higher total lipid dose in a distinct LNP formulation: Individual lipid molecules cannot be considered equivalent to the kinetically trapped supramolecular LNP structure. The biological behavior of an LNP is not simply the sum of the properties of its individual lipid components. Rather, LNPs represent dynamic supramolecular assemblies in which lipid composition, molecular interactions, packing behavior, and spatial organization determine emergent properties at the particle level. Conversely, the supramolecular environment of the LNP alters the behavior, accessibility and biological interactions of the individual lipid components. Therefore, biological activity cannot be inferred from lipid mass or individual lipid identity alone, but requires evaluation of the complete nanoparticle architecture.

Furthermore, patisiran(Onpattro®) is administered intravenously together with premedication including H1 and H2 blockers in a high-risk patient population in which subtle long-term safety signals would be inherently difficult to detect epidemiologically. In addition, Patisiran contains a fundamentally different RNA cargo (siRNA rather than mRNA) and, importantly, is based on a distinct LNP formulation with different supramolecular organization and physicochemical properties. These differences include lipid composition, particle characteristics, surface properties, biodistribution and intracellular trafficking behavior, all of which can influence biological interactions independently of the RNA cargo itself. Therefore, safety observations from Patisiran cannot be directly generalized to mRNA-LNP platforms without demonstrating that the relevant LNP-mediated biological properties are comparable.

Bates et al. (2023) demonstrated another important difference in patisiran(Onpattro®): The authors showed that the rate of endosomal escape is important and Onpattro is far less aggressive. This in turn leads to less galectins, NLRP3 activation, etc.

Furthermore, the regulatory evaluation of LNP-based mRNA vaccines itself reflects the unresolved complexity of assessing these systems: In a public CBER/FDA workshop presentation, Keith Peden (Division of Viral Products, October 2022, “Considerations for the Quality, Safety and Efficacy of Prophylactic Lipid Nanoparticle mRNA Vaccines”) explicitly listed as one of the “Possible Safety Issues” the unresolved question: “Should the individual LNP components be evaluated separately or as the vaccine?”

In addition, within the context of chemistry, manufacturing and controls (CMC), the presentation noted that “there are no settled specifications” for critical product attributes. This is highly relevant when interpreting safety data from different LNP platforms. If the contribution of individual LNP components and their supramolecular organization to biological activity and safety assessment remains an unresolved regulatory question, then safety observations from one LNP formulation cannot simply be extrapolated to another structurally distinct LNP system.

Therefore, the absence of a clear safety signal in patisiran(Onpattro®) cannot be interpreted as evidence that the LNP systems used in mRNA vaccines possess equivalent biological properties, particularly when the ionizable lipids, particle architecture, way of administration, biodistribution, stochastically contributed protein corona, and RNA cargo (which should be understood as part of the assembly) differ substantially."

Dear Michael, Thank you for taking the time to read our review. I think we fundamentally disagree on what the study by Marks et al. is actually able to demonstrate. The experimental design of Marks et al. is centered on RNA delivery, antigen expression…

"Muscle fibers have been implicated in the efficacy of i.m. DNA and lentiviral vaccines50,51,52,53,54, with extracellular vesicle (EV) release implicated55. However, it is not clear why antigen in myocytes would have such a strong impact on increasing the frequency of CD8 T cells (~30–50%) when there is so much direct expression in pAPCs from RNA-LNP injection. A possible explanation is that myocyte expression serves as a source of exogenous antigen for cross-presentation by DCs and this is a more potent mechanism of CD8 T cell activation than direct presentation by transfected DCs, as suggested by flu studies47,48. It may also be that skeletal muscle retains antigen for longer and can serve as a reservoir of antigens to enable more continuous expansion of CD8 T cells."

"An unanswered question is how antigens are transferred from transfected to nontransfected pAPCs. This could be through cell death (releasing free antigens or apoptotic bodies carrying antigens) or potentially through EV release. Surface shedding is another possibility and may account for why the T cell response to Spike and OVA, which are membrane bound, was less dependent on pAPC expression than GFP, which is cytoplasmic. Cross-dressing, involving transfer of peptide–MHC-I complexes from transfected cells to pAPCs, could be another mechanism, and suggested by recent work. More studies will be needed to determine this but our findings have implications for RNA-LNPs that are not intended for vaccination, as avoiding expression in APCs may not be sufficient to prevent immunity against the payload. Although the reduced magnitude of the response with intracellular antigen (GFP) suggests that it is still beneficial to include 142T in nonvaccine applications of mRNA therapeutics, such as for Cas9 expression, to detarget from pAPCs."

Punk 2
